Requesting an Appointment
How to schedule your appointment
Scroll down this page and select the “Request an Appointment” button. A new window will appear with a request form to fill in. Available hours for Dr. Albritton and Dr. Goldsmith are listed at the top of the form, so you are able to request a certain time based on which doctor you’d like to see. Once you have gone through and filled in all fields with your information, select the “Submit” button. When your request has been submitted we will contact you to confirm details about your appointment. If your requested date and time is unavailable, we will work with you to find another convenient time.

Preparing for an appointment
Please bring your insurance card and photo ID to your appointment. It is helpful to bring any medication list, images and reports you may have that are relevant to your diagnosis. Note you will have to pay for parking.
Physician referral requirements
Physician referrals are only necessary if your insurance company requires one. If you are unsure whether a referral is needed, please contact your provider prior to your appointment.
